Lab results will not be given over the phone. All lab results will be reviewed at your follow-up visit. At the time of your blood draw, schedule a follow- up visit to discuss your lab results. No results will be discussed with any person other that the patient unless a written consent has been signed to release medical information. This policy has been designed to protect your privacy and to maintain compliance with Federal government guidelines.
All radiology reports will be discussed at the follow-up visit. Please call to notifiy us the day your test is performed and the schedule a review of the results 7 days after your x-ray. Check with your insurance company regarding co-payment and location of participating facilities for your radiographic test.
Forms and Letters:
A $50.00 service fee is required for the completion of all forms/letters the fee may be waved if included as a part of a scheduled appointment
Treatment of a Minor:
All minors MUST be accompanied by a parent or guardian. When accompanied by anyone other that the parent or guardian a written consent for treatment of a minor is requires. Please request this form when scheduling an appointment for a minor
A referral to a specialist is based upon medical necessity. All referrals performed by our office are faxed to the specialist or diagnostic center. You will be notified by phone at your designated contact number with the name and location of the specialist or diagnostic center within 7-10 business days. If you have not received your referral within 10 working days please contact our office. Contact our office to inform us when you have seen the specialist and to schedule a review of their recommendation.
NO ANTIBIOTICS OR NARCOTICS will be called in. You must schedule an appointment for these medications.
Request a refill of all medications at the time of your office visit.
All telephone calls or faxes for refills of medications will be completed in 24 hours after authorization by the physician.
If you have chronic illness which require close monitoring, (diabetes, hypertension, or hyperlipidemia) remember to call for refills 1 week before your medication runs out.
Refill request made Friday, Saturday, or Sunday will be filled the following Monday or next business day.
Refill request made during a holiday will be filled during the next business day.
If you have not yet received your medication or received a notification call after 72 hours you may call our office.
Full payment is due at the time of service. Contact your insurance company to find out your co-payment amount and office coverage. If for any reason you are unable to make a payment, please discuss it with our office manger in advance of your appointment.
In case of an emergency please call 911. Our office is unable to provide life- threating medical attention.

Please notify all ER personnel that Dr. Powell is your primary physician.
Calls after Practice Hours:
Dr. Powell or an on-call physician is available for URGENT medical concerns. All calls made to the office after hours will be forwarded to Dr. Powell or an on- call physician. Expect a return call within 30 minutes. Leave a complete name and contact number with the service. For all non-urgent matters (i.e. referrals, appointments, results) please call back during regular business hours.
